If you require your clinician to complete a support letter, report or form (excluding reports completed as part of a formal assessment, eg. Cognitive Assessments) please complete the request below and we will pass this on to your clinician to complete.

Due to the high volume of requests and the time required to complete these, our standard turnaround time is 21 days. Should you require this sooner, a surcharge will apply as detailed below.
